I had much fluid retention despite lymphatic massages, compression garments arnica teas and supplements. From early post op I had much pain at belly button area which I expected as post op pain. The much fluid in my abdomen has become hard as plastic, very stiff and painful to due anything that involves moving my torso or lifting a gallon of milk. I haven’t been able to resume my runs, even fast walking is very painful and only getting worst. My stomach seems to be bulging out more to the right and swells as the day goes on. The more swelling the more pain. I highly suspect a pseudobursa but my surgeon dismisses all my concerns, ultrasound that I requested shows 2.5mm hypoechoic area yet the surgeon says no surgical intervention is warranted. Therefore dismissing all my concerns, no regards for my pain. Refuses to take me back to surgery to find the source of this pain claiming it would be malpractice without the evidence but he has the evidence in the ultrasound. He is now demanding I get an MRI, knowing very well the MRI will not show scar tissue so he can send me off on my merry way with a negative impression on the MRI. The pain and the ugliness of the bulging have drastically changed my life, I am physically, emotionally and mentally in pain. My world came to a crashing halt today when his NP said to me, no surgical intervention is needed, then that’s it?? I was some experiment he screwed up and now tosses me to the curve? I am 110% sure I have a pseudobursa, why does he insist on dismissing my symptoms? Dr Repta, will you please take a look? I ended up with a pseudobursa on my left side just above the incision along with multiple very small (1cm or less) encapsulated seromas on my right side positioned similarly above the incision. This May, I had a scar revision to remove the offending pseudobursa and the small encapsulated seromas.

Even though there was an initial seroma issue where the pseudobursa had been, it was frequently drained and eventually dried up after about two weeks of daily aspirations. All was coming along.

Suddenly 3 to 3.5 months after the revision, I began to experience chills during the night. The next morning, my lower abdominal muscles felt tike they were burning and intense, hard edema covered my entire lower abdomine along with fever a d disorientation.

I went to the ER and after a CT scan noticed several small fluid pockets on the abdominal wall. The medical team surmised they could be abscesses and causing the symptoms. Over the last three days, I have received many treatments of IV antibiotics. Regardless, I am not well, the pain is severe, the muscle is cramping and the hard swelling persists. The medical team at the hospital seems to be somewhat lost as in what to do.

The redness is growing, the pain is increasing and I want to be healed. Please give me some insight I can pass on to them as I am scared and desperate.

Kindest regards,

CVAnonymoushttps://www.blogger.com/profile/07608200903014724820noreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-8920517965204040162020-05-19T11:11:19.014-07:002020-05-19T11:11:19.014-07:00I have such horrible scar tissue that I can no lon...I have such horrible scar tissue that I can no longer stand for long periods of time or enjoy nice, long walks. Now it's is 2020 and I really do think I have this Pseudobursa scar tissue. I constantly get gastric issues, bloatness and muscle tightens across my stomach. Sometimes it swell when I exercise. My stomach feel like I have a thick layer of skin underneath that is not responding to exercise. The tightness is not so bad while sitting. I have had fascia deep tissue massages to try on losing this feeling.I get spasms and wear a waist band all the time to reduce the feeling. Initially I was just going to try in live with this, but I started thinking do I really want to be in this same situation 30 years from now. The right side is harder than than the left and I still massage my stomach throughout the day. I am San Diego and would love to either find a doctor local or arrange for a consult with you. All the exams show scar tissue. I had my gallbladder removed in May 2019 and my doctor said it was hard going thur my belly button because of the scarring,(2 hernia repairs) I am fit, and would love to get this fixed but I am not looking forward to spending another 10k or more to get this repaired plus the pain of it all. Any comments are welcome. First of all I was supposed to be the only surgery that day and got pushed back the day before for a facelift. Surgery was done in an outpatient facility. The surgery began about an hour later than scheduled. I didn't have a good feeling about it but what do you do. I was committed at that point or loose my money. That being said I voiced my concerns about the bulge in my upper abdomen from the beginning. The dr. made a "joke' with the nurse and said maybe I tied the lower sutures too tight. I didn't see the humor. He proceeded to tell me I needed to make an appt w/a gastrologist. I went back for several followups. BTW he did not use drains or mesh, he said if you do it right it wasn't necessary. To summarize the comments I have to endure: I am almost 60, its fat and I need to lose weight. The surgery "revealed" my shape (which looks like a balloon animal) Even his nurses agreed with me and said that they see it in my pictures. He said he wished he could have seen a CT scan pre surgery. I had my first child when I was 18 and had a vertical c section (back in the day) and alot of stretch marks. Still have stretch marks and now scars and a tumor looking hard and uncomfortable mound in my upper abdomen. I waiting decades to finally feel comfortable in my body as a woman. Now I am left with snide remarks and a less than desirable result. Never ONCE did he mention draining anything. So discouraged. :(LJGhttps://www.blogger.com/profile/04767270590457696851noreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-41401610799637834862019-02-01T03:30:32.023-08:002019-02-01T03:30:32.023-08:00Oh, I forgot to add that after my tt, the area rig...Oh, I forgot to add that after my tt, the area right above my belly button is hard and sticks out. The PS had to make an incision to clean the infected area out & I packed the area, using Dakin's solution after each dressing change. This area is now hard and flat. It almost feels like it interferes with my digestive area and like I need to suck in my stomach all the time, walking and standing up super straight, my back hurts and sometimes when I bend over the area "stabs" me. Leaning on things and sitting provide comfort & forget walking long distances! It's so uncomfortable. Is there anything that can be done? I just want to be free from this binding feeling. Thank you for your blog! I just stu...Hi Dr. Repta,

Thank you for your blog! I just stumbled upon it and find the information invaluable. And thank you in advance for entertaining my questions!

Does a pseudo bursa show up on ultrasound and/or CT scans?

I'm 15 months post extended abdominiplasty with muscle repair and lipo. I am thrilled with my cosmetic results! I had a pretty normal recovery, but also did have a seroma needle aspirated in my surgeon's office. But I have now developed a large hard "mass" with pain in my lower abdomen above my scar line, in the area between my pubis and bellybutton.

My PCP has suspected adhesions because I've had 4 abdominal surgeries, 3 laparoscopic surgeries years ago, and the TT in November 2016. But since the only way to confirm adhesions is surgically, we agreed on a course to rule out other things and work up from less invasive diagnostic tests. It wasn't so bad when we started months ago, but during that time, it seems to have gotten worse and larger. The discomfort is more acute and steadily present, and tends to get worse as the day wears on.

In addition to other diagnostic tests, I've had two pelvic/abdominal ultrasounds (trans abdominal and trans vaginal), and both were unremarkable. And lastly, I had a CT with contrast, and it was normal with the mass not showing up. It is my understanding that scar tissue doesn't show up on imaging. Is that right?

My PCP has referred me to a general surgeon for likely exploratory surgery, and I'm awaiting an appointment for assessment. I recently stumbled upon information on pseudo bursa, and it sounds exactly like what I'm experiencing. At its worse, it feels like a tight band across my lower abdomen. But I've seen comments that sound like they were present on imaging studies. Is that right? Do they show up on ultrasound/imaging? Is it unlikely that I have this if my tests were clear?

Thank you!
BelindaBelindanoreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-87335645413381636232018-01-29T06:58:54.494-08:002018-01-29T06:58:54.494-08:00Hello Raecrz,
The area above the belly button is ...Hello Raecrz,

The area above the belly button is the second most common area for a pseudo bursa after the areas between the incision and the bottom of the belly button.

Since you had a longstanding seroma in that area it is likely that what you are seeing and feeling is associated with the formation of a pseudo-bursa.

Trying to access the pseudo-bursa through the belly button incision is not recommended since all of the pseudo bursa should be removed. The central part of your TT incision should likely be used. It does not have to be the entire hip-to-hip incision but it should be long enough to give the proper access needed.

Dr Remus ReptaAnonymoushttps://www.blogger.com/profile/17525644549512347584noreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-9645435120776575912018-01-29T06:54:43.985-08:002018-01-29T06:54:43.985-08:00Hello D A.

This does not mean that the whole entire prior surgery must be redone. A portion of your TT incision would have to be opened to gain access but the recovery would be significantly easier than what you have experienced before.

It will be important for both layers of the pseudo bursa to be removed. This takes a little more thought and time in the operating room but its well worth it.

Dr. Remus ReptaAnonymoushttps://www.blogger.com/profile/17525644549512347584noreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-58015514062820611052018-01-28T15:54:12.064-08:002018-01-28T15:54:12.064-08:00Hello Doctor,

It is between the scar and my bellybutton. It feels as if it is swollen or some sort of artificial slab that is underneath my skin. It is quite annoying to deal with as I did not have this before.

I saw a PS and he said he could liposuction it but how will I know if it is fluid retention, pseudobursa or something else? He just seemed to burst out a solution without too much focus on it.

What do you recommend I do? I collected fluid there and it was drained 2 times, the second time there was much less fluid. my surgery was out of the country so when I came back to the USA I had no one to drain it for me. I saw a surgeon because I had a small complication with my incision so I showed him the bulge at the top of my belly button and he said that my body will get rid of the liquid and he didn't want to drain it. well its been 9 months since surgery and I still have the bulge. its very small it goes from the side to the top surrounding my belly button on my right side. you can feel it more that you can see it but when I lay flat its more noticeable. basically my question is, being that its so small I wanted to know if there's any other option that doesn't require opening my entire scar from hip to hip up again? or can they open just the center of my scar?. also I like my bely button very much if I have to have surgery to fix this will they have to touch or redo my belly button? and lastly, the bulge is not bigger than maybe a small meatball could this be liposuction? thanks Anonymousnoreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-69388331795928473252017-05-10T09:31:53.284-07:002017-05-10T09:31:53.284-07:00Hello Anonymous hematoma removal at 4 months,

Adding quilting sutures and a drain should decrease the chance of seroma formation. Since a seroma is healing fluid technically everyone is a seroma former. The key is to drain the seroma so that the tissue heal together. Once a pseudobursa has formed the tissue cannot heal together as easily and removal is often needed.

Happy to help however I can.

All the best,

Remus
Anonymoushttps://www.blogger.com/profile/17525644549512347584noreply@blogger.comtag:blogger.com,1999:blog-8140316791707391928.post-21649263210720354832017-04-25T16:06:12.595-07:002017-04-25T16:06:12.595-07:00Sorry I didn't sign in with my email.

One in my right upper quadrant and one on each posterior Iliac Crest. They where drained 10 times by an Interventional radiology. The last drain was in November. I have been feeling very sore in my butt area for the last 3 months. I had an ultrasound done on Monday and they all have fluid again. The one in the front has a minimal amount and doesn't cause much discomfort. The one on my back right side has calcified with minimal fluid, it is hard as a rock and I have extreme pain that radiates towards the back. The one on the left back side has a good amount of fluid but not as much calcification. This side also is extremely painful. My PS wants me to get the left one drained again with adding a saline mixture to get it to heal. He wants to inject steroids into the right side to break up the calcification. He said to massage the areas to help with the break up.
I asked about removing the capsules and he was concerned the if he removes them new pockets could form. He considers me " Seroma Former". I know he has my best interest in mind but I would really like an outside opinion. Also, If you believe the seroma should removed should it be done under ultrasound guidance? I thought that when it was removed the tightness and pain would be gone..I still have extreme tightness and pain where the hematoma was. I'm pretty sure that not all of the encapsulated hematoma was removed. My concern is if I have a surgery to remove the remainder of scar tissue, what are my chances of the scar tissue coming back? Is it more likely that I will be cured of this scar tissue nightmare?

Anonymousnoreply@blogger.com
